Dropsy isn’t actually a disease — it’s a symptom. Specifically, it’s the result of fluid accumulating inside a fish’s body cavity and tissues when the kidneys and other internal organs begin to fail. That fluid has to go somewhere, and it pushes outward: bloating the abdomen and forcing individual scales away from the body at an angle. The result is the classic “pinecone” appearance that makes dropsy unmistakable once you know what you’re looking at.
The underlying cause in virtually all cases is a bacterial infection — most commonly Aeromonas bacteria, which are present at low levels in nearly every freshwater aquarium. Under normal conditions, a healthy fish’s immune system keeps these bacteria in check. Dropsy happens when something has already knocked that immune system down, giving the bacteria an opening to cause serious internal damage.
A bloated belly alone doesn’t automatically mean dropsy. The scale-raising is the diagnostic hallmark. If your fish looks swollen but the scales are lying flat, you may be dealing with something else entirely.

What Causes Dropsy in Fish?
The bacteria that cause dropsy — primarily Aeromonas — are already living in your tank. They’re not something you introduced. What you introduced, at some point, was a level of stress that compromised your fish’s immune system enough to let those bacteria take hold internally.
The most common environmental triggers include:
- Ammonia or nitrite spikes. Any detectable ammonia is dangerous. During a tank cycle or after a filter crash, fish can be exposed to toxic levels for days before an owner notices. That kind of chemical stress is an open door for bacterial infection.
- Chronic poor water quality. Nitrate creeping above 40–50 mg/L over months, skipped water changes, and decaying organic matter all suppress immune function gradually.
- Sudden temperature swings. A failing heater, a cold water change, or a tank placed near a drafty window can stress fish enough to trigger illness.
- Overcrowding and aggression. A fish that’s constantly harassed by tankmates is a fish under permanent immune stress.
- Poor diet. Fish fed a low-variety diet or stale food miss out on vitamins (particularly Vitamin C) that support immune function. A fish ignoring food at feeding time is one of the earliest signs something is wrong.
The fish that developed dropsy was telling you something had been wrong in the tank environment for a while. That’s not a judgment — it’s useful information for prevention, which we’ll get to at the end.
Is Dropsy Contagious to Other Fish?
The short answer: not the way ich is contagious. Dropsy isn’t going to jump from a sick fish to a healthy fish that’s thriving in clean water. The Aeromonas bacteria responsible are already present in most established freshwater tanks at low background levels — your healthy fish are already coexisting with them without getting sick.
The risk to tankmates isn’t direct transmission — it’s that if one fish got dropsy, it means the tank environment is stressful enough that other fish are also immunocompromised. They’re all living in the same water. If ammonia is spiking or nitrate is chronically elevated, every fish in that tank is at elevated risk.
The right response is to quarantine the sick fish immediately (more on that below), then test and correct the water quality in the main tank for everyone else. Don’t dose antibiotics in the main tank — antibiotics kill the beneficial bacteria in your biological filter, which will cause the water quality problems that led to dropsy in the first place to get even worse.
Dropsy Survival Rates — Be Honest With Yourself
